Online scheduling makes booking your appointment convenient and easy. If you prefer, feel free to give us a call or text and we can find a day and time that works best for you.

Intake Forms
Once your appointment is scheduled, you will receive a New Patient Intake form in your email inbox. Please have this completed prior to your appointment to allow for a smooth first visit.

First Visit
The first visit is a 40 minute appointment. During this appointment we will sit down and take a detailed history, go through a movement assessment and diagnostic testing to get to the root cause of your injury, and provide treatment to help relieve your symptoms.

Clothing
For your first visit we ask that you wear comfortable clothing that is non-restrictive and allows for full examination and treatment of the affected area. Consider wearing shorts, sports bra, open back/neck shirt, or a short sleeve/tank top shirt.

Follow Up Visits
After your initial visit, follow up visits are scheduled for 10 or 20 minute time increments. Most visits include a combination of adjustments, myofascial release, dry needling, therapeutic laser, and/or rehabilitative exercise.

Payment
We accept all forms of payment including cash, credit card, and HSA/FSA card. Payment is due at time of service. An itemized receipt can be given for any out-of-network benefits or verification of services provided. We are not in network with insurance.
